Biggest problem I repeatedly come across client after client working as an accessibility consultant. The sheer volume of outright spite some people have for the "annoying cripples" that they "shouldn't have to learn to support" is the number one thing that makes me feel burmt out.
But it goes further than that as I often come across people who treat their business as some sort of dodge, or hustle. Which is bad when dealing with banks, public utilities, and medical clinics.
